案件進度查詢
依據預防接種受害救濟基金徵收及審議辦法第6條規定，申請受害救濟應填具申請書，本系統收到您所提供的登錄資訊後，
將由公衛人員與您聯繫，協助您完成申請程序。在您登錄前請注意以下事項：
This is the report system for suspected vaccine injury cases. According to Article 6 of the Regulations Governing
Collection and Review of Vaccine Injury Compensation Fund, a claimant who files a claim for vaccine injury
compensation shall submit a Vaccine Injury Compensation Application. After receiving your report, public health
workers will contact you for further investigation and assistance in claim submission. Before you fill out the
report form, please read the following instructions.
1.接種疫苗後發生不適症狀者，請先行就醫治療，避免延誤治療時機，且相關治療病歷將成為案件鑑定必要資料。
If you are experiencing any adverse events or severe side effects after getting vaccinated, please seek medical
attention immediately for prompt diagnosis and treatment.
Your medical record will serve as essential information for case determination and review.
2.請務必提供真實且可供聯絡之資訊，以利及時與您聯繫，並協助您可完整提供鑑定與審議所需之相關資訊(包含於紙本申請書簽名)
，以完成申請程序，維護您的權益。
To protect your rights, and to assist you in preparing documents (including signature on the claim application)
needed for the determination and review process and to file a compensation claim, please make sure to
provide correct contact information for us to reach you in time.
3.待公衛人員協助完成申請程序，您將收到社團法人國家生技醫療產業策進會寄送之受理通知，之後可點選右上角「案件進度查詢」功能查詢案件進度。
After public health workers file your compensation claim, you will receive the notification of acceptance from the Institute
for Biotechnology and Medicine Industry. Then you can click “Check Case Status” at the upper-right corner to
check the progress of your case.
